This is my online custom shoot request form for specifying custom shoot/video requirements.

Note that items marked with a are mandatory.

NOTE: I no longer model other than for my site hence please don't ask for modelling services as a refusal often offends. All custom shoots are shot in-house using my own photographer/videographer hence if you are after shooting the material yourself I would urge you to look elsewhere.
